Case management services are a free Medicaid benefit that:
Helps eligible clients access medical and educational services.
Enables patients to follow through on medical recommendations.
Prevents new health problems or worsening of existing problems.
Optimizes healthcare outcomes.
Eligibility checklist
Is the client:
Eligible for Medicaid?
A child, ages 0 - 20, with a health condition or health risk?
A woman with a high-risk pregnancy?
In need of services related to the health condition/risk?
Note: You don't need to determine eligibility. If a client appears to have a need for case management services and appears to be eligible, you should make a referral.
Examples of case management services
Finding a doctor and scheduling appointments.
Linking to developmental testing.
Addressing school needs.
Linking to services for financial assistance or transportation in cases of pregnant women facing eviction or having difficulty attending appointments due to lack of transportation.
Finding other resources, such as respite.
Coordinating DME, home health nursing, etc.

Anyone in your office can refer a client for case management services. To make a referral, either:
